Instructions for Commenting on the Program
The ORAU Discussions blog runs on WordPress.com’s blogging software and is designed to provide a user friendly experience. Content posted on the blog is intended to provoke discussion and comment.
To leave your comments, you will need to click the “Comments” link following the last paragraph of the content. You will then see a blank field labeled “Leave a Reply” where you can begin typing. Depending on your view, you may see the “Leave a Reply” box without clicking the Comments link.
When completing this form, please include your name, your position, and the name of your insitution. While this information is not required, if you do not include it, we will not have any way to know who is responsible for each comment. If you would like to be contacted for follow up, please be sure to leave your e-mail address.
Instructions for Forms
NOTE:You must have Microsoft Word 2003 or higher in order to complete the forms electronically. If you have trouble, please send an e-mail to hpcgrant@orau.org for help or alternate instructions.
- There are two forms that must completed. Use the following links to download the Certifications form and the Contact and Demographic Information form.
- Fill out the forms by clicking in the text box beside/below the question and typing in your answer. Certain text boxes will expand to accommodate the amount of text you type into them.
- For those questions that require you to check a box, click on the box that corresponds with your selection, and an “X” will appear in the box. If you change your mind, click on the box a second time to deselect it.
